Dignity and choice at the centre
People choose how they are supported, by whom, and in what way. Consent, cultural respect, faith, gender preference and personal routines guide every plan we build.
Family members and trusted professionals are welcomed as partners when the person we support wishes to include them.
How we intend to deliver complex care
- 1
Multi-disciplinary planning
Care planned in close partnership with GPs, community teams, therapists and specialist nurses.
- 2
Specialist training
Care teams trained specifically for each person's condition and equipment needs before support begins.
- 3
Structured governance
Careful clinical oversight, record-keeping and escalation — built to satisfy CQC expectations from launch.
- 4
Family partnership
Families are welcomed as experts in their loved one, involved fully in planning, reviews and daily updates.
Future assessment process
- 1An initial conversation to understand what matters to you.
- 2A home visit to assess environment, safety and preferences.
- 3A written support plan agreed with you and, if you wish, your family.
- 4Introductions to your consistent carer team before support begins.
- 5Regular reviews to keep the plan aligned to your goals.
Family and professional involvement
With consent, families, GPs, social workers and other professionals are welcomed into planning and review. Clear communication, safeguarding and record-keeping will be central to how we work.
We are designing our operational model to meet CQC expectations from the moment regulated services begin.
