- Stronger social connections and reduced isolation
- A clear voice in how support is delivered
Dignity and choice at the centre
People choose how they are supported, by whom, and in what way. Consent, cultural respect, faith, gender preference and personal routines guide every plan we build.
Family members and trusted professionals are welcomed as partners when the person we support wishes to include them.
How we intend to deliver supported living
- 1
Person-centred planning
Support is shaped around aspirations — from cooking a favourite meal to holding down a job — not just tasks.
- 2
Skills for independent living
Practical coaching in cooking, budgeting, travel, health appointments and household routines.
- 3
Positive risk management
Enabling choice while managing risk carefully in partnership with families and professionals.
- 4
Community and belonging
Support to build friendships, hobbies and community roles — a life, not just a service.
Future assessment process
- 1An initial conversation to understand what matters to you.
- 2A home visit to assess environment, safety and preferences.
- 3A written support plan agreed with you and, if you wish, your family.
- 4Introductions to your consistent carer team before support begins.
- 5Regular reviews to keep the plan aligned to your goals.
Family and professional involvement
With consent, families, GPs, social workers and other professionals are welcomed into planning and review. Clear communication, safeguarding and record-keeping will be central to how we work.
We are designing our operational model to meet CQC expectations from the moment regulated services begin.
